CBT, DBT and ACT in plain words

The short answer

What is the difference between CBT, DBT and ACT?

CBT (cognitive behavioural therapy) teaches you to spot and change thoughts and habits that feed anxiety or depression, usually in 12 to 20 structured sessions. DBT (dialectical behaviour therapy) adds skills for intense emotions and crises, often in a year-long group-plus-individual program. ACT (acceptance and commitment therapy) teaches you to make room for hard feelings and act on your values.

The first time I read about therapy types, the alphabet soup nearly made me close the tab. CBT, DBT, ACT, and a dozen more. It sounds like a menu where you're supposed to already know what you want.

Here's the plain version. CBT helps you catch the thoughts and habits that keep you stuck, and change them. DBT is CBT's tougher cousin, built for people whose emotions go from zero to a hundred, with skills for getting through a crisis without making it worse. ACT says stop wrestling every bad feeling to the ground, make room for it, and do what matters to you anyway.

None of them is "the best." They're tools for different jobs. And a lot of therapists mix them.

What is CBT?

Cognitive behavioural therapy is the one you've most likely heard of. CAMH describes it as structured, time-limited, problem-focused and goal-oriented. For anxiety and depression, it usually runs 12 to 20 sessions. Some people get a lot out of four to six.

The basic idea: what you think about a situation shapes how you feel and what you do. So you learn to notice the thought ("they didn't text back, they hate me"), check it against the evidence, and try another way of seeing it. There's homework between sessions. Logging thoughts, trying the thing you've been avoiding, that kind of stuff.

It's used for a long list of things: anxiety, depression, OCD, panic, PTSD, substance use and more.

My honest take: CBT works well for a lot of people, and the homework is where it happens. If you're an argue-with-the-worksheet kind of guy (I am), you'll either love it or fight it for a month first. If you're autistic, ask how the therapist adapts it. Some of the "is that thought realistic?" stuff assumes you read social situations the way most people do, and sometimes your thinking is fine and the room genuinely is confusing.

What is DBT?

Dialectical behaviour therapy was developed for people with borderline personality disorder, and it has since been used for suicidal behaviour, self-harm and substance use. "Dialectical" just means holding two things that seem opposite at once. The big one in DBT: accept yourself as you are right now, and change.

DBT teaches four sets of skills:

  • Mindfulness, to notice what's happening before you react.
  • Distress tolerance, to get through a crisis without making it worse.
  • Emotion regulation, to understand and turn down intense feelings.
  • Interpersonal effectiveness, to ask for what you need and say no without blowing up the relationship.

A full DBT program is a real commitment. CAMH describes weekly skills groups (about two and a half hours), weekly individual therapy, phone coaching for real-life moments, and usually a one-year commitment.

This is the one that makes me think hardest about my own past. Robplosion is the word a friend came up with, almost twenty years ago, for the way I'd explode at people when problems came up. Distress tolerance is pretty much a manual for the ten seconds before that happens. The workbook below is the one I'd hand someone first. A workbook isn't the full therapy, though. If you want real DBT, the DBT-Linehan Board of Certification lists certified clinicians and programs, mostly in the US. In Québec, ask your CLSC or a psychologist directly whether they offer DBT or DBT-informed skills groups.

What is ACT?

Acceptance and commitment therapy, usually said as the word "act." The Association for Contextual Behavioral Science describes it as using acceptance and mindfulness, along with commitment and behaviour change, to build psychological flexibility. Steven Hayes is the name most tied to it.

In plain words: you stop treating every painful thought as a problem to solve or an enemy to beat. You learn to let it be there, notice it's a thought and not an order, and then do the thing that matters to you anyway.

Where CBT might ask "is that thought true?", ACT asks "is holding on to it helping you?" Different door, similar room.

I like ACT for people who've done years of trying to think their way out and are tired. Some of us have argued with our own heads since childhood. ACT says you can stop arguing and still move.

Which one is right for me?

Rough guide, not a rule:

If you're dealing with anxiety, depression, panic, or a specific fear, CBT has the most research behind it and is usually where people start.

If your emotions hit like a truck, if you've had crises, self-harm, or explosions you regret, DBT was built for that.

If you're worn out from fighting your own mind, or living with something that won't fully go away (chronic pain, long-term depression, autism-related stress), ACT might fit.

And honestly, the relationship with the therapist matters as much as the letters. A good therapist will explain what they do and why. Ask. I wrote about finding an autism-aware therapist in Québec and what happens in the first session.

Can I get these for free in Québec?

Sometimes. Québec's public network uses a stepped approach, and the services listed include individual interventions using CBT techniques, group interventions and psychotherapy. Start at your CLSC or call Info-Social 811. You may not get to choose the approach. Private psychologists usually list theirs in the Ordre des psychologues directory. Questions people ask

What is the main difference between CBT and DBT?

CBT focuses on changing unhelpful thoughts and behaviours. DBT keeps that, then adds acceptance and a set of skills for intense emotions and crises. DBT is usually longer and often includes a weekly skills group.

Is ACT better than CBT?

Neither is better across the board. They come at the problem differently. CBT works on changing thoughts. ACT works on changing your relationship to them. Some people click with one and not the other.

Is DBT only for borderline personality disorder?

No. It was developed for BPD, but CAMH notes it's also used for suicidal behaviour, self-harm and substance use. The skills are useful for anger and emotional overwhelm whether or not you have that diagnosis.

Do these therapies work for autistic adults?

They can, especially when the therapist adapts them. Ask how they adjust for autistic clients. If the answer is "I don't," keep looking.
